In most vintage Ballpoints, the refills are dry and have to be replaced.

The Parker pen cap action for ballpoints wasn't the first design. Parker's early ballpoint pens, like the Jotter, launched in 1954, featured a mechanical click-action to advance the refill. This "cap action," where the tip is advanced by pushing down the cap, allowed for a smooth and quick extension and retraction of the writing tip. This design replaced older, more complex mechanisms and was a key feature for the Jotter, which remains popular today. 

The Parker Style G2 refill is the most common size and shape on the market. Nearly every pen and/or refill maker offers a refill that matches this standard. . The GEL version of this refill is available as a medium point in black, blue, or red ink. As previously mentioned, numerous third party refill makers such as Jaymo offer regular and GEL substitutes in many line widths and colors.
